Shoulder pain can be manageable during the day but turn into a deep, nagging ache when you lie down. Here’s why it often worsens at night—and what you can change to sleep better.

Why Shoulder Pain Feels Worse at Night

1) Compression + Position

Sleeping on the painful shoulder compresses irritated tissues. Even lying on the opposite side can pull the sore shoulder forward.

2) Inflammation Builds Over the Day

Daily activity causes micro-irritation. Overnight, blood flow rises for repair—this can temporarily increase soreness.

3) Muscles Relax

At night the muscles supporting your shoulder fully relax, reducing stability and irritating sensitive structures.

4) Common Causes

Rotator cuff irritation, bursitis, referred neck pain, or frozen shoulder often contribute to night pain.

Quick win: Avoid falling asleep on the painful shoulder or with the arm overhead.

Best Sleeping Positions

  • On your back: Small pillow under elbow.
  • On the opposite side: Hug a pillow to support top arm.

Avoid: Stomach sleeping, lying on painful shoulder, overhead arm position.

Simple Daytime Exercises

  1. Pendulum circles: 30–60s.
  2. Band external rotations: 2–3×12.
  3. Pec doorway stretch: 2×30–45s.
